Florida Committee on Trauma Membership

To become a member, please complete the 2018-2019 Membership Form (PDF) and return it with payment to the address or fax number on the application form.

Types of Membership:

• $100 Active Member - Active membership is open to Fellows of the American College of Surgeons (FACS).
• $100 Associate Member - Associate membership is open to medical doctors who are non Fellows of the ACS.
• $100 Advisory Member - Advisory membership is open to healthcare providers in trauma centers and hospitals in the State of Florida who are non physicians.
