Trauma Center Emergency Contact Information

In light of Florida’s natural disasters and manmade catastrophes of recent years, disaster preparedness and resource sharing among trauma centers to ensure the safety of Florida’s citizens and visitors is a top priority for the Florida Committee on Trauma (FCOT). FCOT is collecting contact information for Florida's trauma centers, trauma medical directors, and trauma program managers to ensure open communication, particularly in a time of need. Please complete the following form.
